CARE MANAGER - The Better Urban Health CANS-NY Certified Care Manager will be responsible for coordinating services for eligible children and youth ages: Pregnancy 0–21, some with serious emotional disturbances (SED), chronic medical conditions, and/or developmental disabilities.

CARE MANAGER SUPERVISOR / SENIOR CARE MANAGER - The Better Urban Health Care Manager Supervisor / Senior Care Manager is a dual-role position that provides both supervisory oversight of Health Home Care Managers and sometimes direct care coordination to a limited caseload of children with high-needs. This role ensures all care management activities are performed in compliance with NYS Department of Health (DOH) regulations, CANS-NY standards, Health Home policies, and NYC Health directives.
