HOW DO I KNOW IF MY CHILD (MY CHILDREN) OR IF I QUALIFY?
In order to be eligible for Health Home care management services, ask yourself the following questions:
- Is my child or any of my children in between the ages of 0-21?
- Am I in between the ages of 18-21?
- Does my child, my children or I need a primary care doctor?
- Am I pregnant?
- Is my child pregnant?
- Does my child, my children or I need Health Insurance?
- Does my child, my children or I have a medical condition that needs to be treated by a doctor?
- Does my child, my children or I possibly have a mental health condition that needs to be treated by a doctor?
- Does my child, my children or I (each) have our own individual doctor that can be seen whenever I or they need to?
- Does my child, my children or I have someone in our lives to help whenever I, we or they need help?
- Does my child, my children or I have difficulty keeping and making it to healthcare appointments?
- Has my child, my children or I visited an emergency room or hospital more than once in the past six months or twice in the past 12 months?
- Does my child, my children or I need help with school network meetings?
- Does my child, my children or I need help understanding DSS, ACS or CPS matters?
- Does my child, my children or I need help getting medication?
- Does my child, my children or I need new affordable housing because my current living situation is unhealthy or unsafe?
- Does my child, my children or I need support with understanding legal documents and/or court proceedings?
- Does my child, my children or I want to be seen by a healthcare provider that looks like us?
