What is Care Coordination and Management?
Care coordination and management is a service provided by Better Urban Health. Once your child is assessed and enrolled, a care manager will be assigned to work closely with the caregiver and the child to address all healthcare needs. Your child's care manager will invest quality time into getting to know you and your child. Your child's care manager will learn your child's current healthcare needs and all future healthcare concerns. She will then coordinate and manage all of your child's physical health, behavioral health, educational health, social health and/or developmental disability healthcare needs. Once enrolled, your child is immediately eligible to receive the following services:
- Connecting to the health care provider of choice,
- Connecting to mental health
- Connecting to substance abuse providers,
- Connecting to needed medications,
- Help with housing,
- Connecting to social services, e.g.: food, benefits, and transportation or,
- Other community programs that can both support and assist you.
